Folks, as a club, we're not responsible for keeping your personal medical info up to date, but if you are out on a ride and get hurt and the guys along with you need to get medical attention for you, it's probably a good idea for everyone to take personal responsibility to print and fill one of these out, fold it up and keep it on your person before your next ride.
When you join the club , you'll get a plastic licence/registration holder (courtesy of North Bay Cycle and Sports) with your membership card. A suggestion might be to keep the medical form tucked in there.
